A care plan is an explicit written document (it may be in the form of a letter) that comes out of a shared understanding of the person’s situation and agreeing the way forward. It should set out what is going to be done, why, when and by whom.
A crisis plan is an explicit written document (it may be in the form of a letter) of the trigger factors/relapse indicators specific to the individual and the additional support/response of all members of the care team, including the individual to these trigger factors/relapse indicators e.g. increase their use of established coping skills; support lines.
Relapse Prevention Plan example
A relapse prevention plan is an explicit written document (it may be in the form of a letter) developed as part of planning for an individual leaving mental health services. It will set out trigger factors/relapse indicators specific to the individual and suggested actions in response.
Wellness, Recovery Action Plan (WRAP®)
A WRAP® plan is a framework with which you can develop an effective approach to overcoming distressing symptoms, and unhelpful behaviour patterns. It is designed by the mental health service user in practical, day-to-day terms, and holds the key to getting and staying well.
